1. Applicant should accomplish the SASD Form (for OFWOSPC use only)
This form is available for free at the Office of Students Services or download it at the CHED Official Website. Below is the replica of the form. Try to download it as your sample before obtaining the official CHED -CAV form.
2. Prepare the following:
 a. Two sets of photocopies of Transcript of Records
 b. Two sets of photocopies of Diploma
 c. Certification from Higher Education Institution
The Office of Students Services is open from Monday to Friday, 8:00AM to 5:00PM. CHED- CAV service fee is only PHP40.00 per copy of the certificate. Maximum of fifteen days is the duration of processing this service.
It is necessary to follow these instructions to avoid confusions and problems.
Applicant should do the following:
a. file the application to the Student Assistant and Service Division (SASD) Staff, Office of Student Services (OSS).
b. pay the Fee to the Cashier.
c. submit the accomplished application form with the attached school credentials and official receipt to the SASD Staff and receives claim stub.
d. follow-up through phone to the Office of Students Services.
·        *  If you are a Sea-based OFW, pick– up the C.A.V. and documents at the OSS.
·        *  If you are a Land-based OFW, follow-up with OSS the Control No. and Houseway Bill 
a        and date of release from DFA.
   Pick-up the CAV and documents with red ribbon upon payment to DFA.
