One of the obligations of every Filipino is to secure their passport if it is about to expire. DFA Passport Renewal Guide has an extensive information to process your passport renewal  plan. If you are planning to renew your passport, you need to read the following details before you will process your passport renewal.

 

Brown Passport or issued prior to 01 May 1995

* Old passport and photocopy of passport pages 1,2,3 (amendment). The pages showing latest Bureau of Immigration departure and arrival stamps.
* Supporting document with complete middle name and Original ID.
* Birth Certificate from NSO in Security Paper (SECPA) or Negative Birth Record from NSO if born 1950 & below

Green Passport or issued after 01 May 1995

* Present passport and photocopy of pages 1,2,3 (amendment) inside and back cover.javascript:void(0)
* The pages showing latest Bureau of Immigration departure and arrival stamps.
* For illegitimate minor, personal appearance of mother is required.

Other Supporting Documents

Please bring the following if they are available.

* Marriage Contract
* PRC / IBP ID
* Land Title
* Driver’s License
* Government Service Record
* Old SSS-E1 form or digitized SSS-ID
* School Form 137 or Transcript of Records
* Other documents that show full name and birth details of applicant and/or citizenship
* Voter’s Registration Record
* Baptismal Certificate
* Seaman’s Book
* Income Tax Return (Old)

If you would like to use the surname of your spouse:

* Marriage Contract (MC) in Security Paper issued by NSO and Certified True Copy issued by the Local Civil Registrar if MC secpa is unreadable.
* Certificate of Attendance from CFO (Commission of Filipino Overseas) if married to a foreigner (color green).

If you would like to revert back to your maiden name.

Please prepare the following as well:

* Death Certificate of the deceased husband and Marriage Contract (MC) in Security Paper issued by NSO or by the Local Civil Registrar duly authenticated by NSO.
* If marriage is annulled: Certified true copy and photocopy of the first page and the dispositive portion of the judgment on annulment and NSO-issued Marriage Certificate with the annotation on the annulment decree.
* If divorced: Certified true copy and photocopy of the first page and the dispositive portion of the judgment on divorce authenticated by the Philippine Embassy or Consulate where the divorce is obtained and NSO-issued Marriage Certificate with the annotation on the divorced decree.

For Minor Applicants

Please take note of the following:

* Personal appearance of minor with both parents or mother .
* Personal appearance of minor with mother (if minor is of illegitimate status).
* Personal appearance of mother (if minor is 7yrs old & below).
* Passport of person traveling with the minor, original and photocopy.

If you are not traveling with either of your parents, follow these instead:

* Original and photocopy of DSWD Clearance
* Affidavit of Support and Consent
* Personal appearance of mother (if minor is 7yrs old & below).
* Passport of person traveling with the minor, original and photocopy

Illegitimate children will need to prepare the following:

* Affidavit Of Support and Consent from the mother
* DSWD Clearance, original & photocopy
* Original ID w/ signature or passport of mother

If you are traveling with your father only, follow these instead:

* Marriage Certificate of parents
* Affidavit Of Support and Consent from the mother, if child is (8yrs old & below)
* Original ID w/ signature or passport of mother

If both of your parents are abroad, follow these instead:

* Affidavit of support and consent indicating the name of the traveling companion and the relationship to the minor (must be authenticated by the nearest Philippine Embassy or Consulate General)
* Special Power of Attorney designating the representative by name and authorizing him/her to apply for a passport on behalf of the minor in their stead (must be authenticated by the nearest Philippine Embassy or Consulate General).
* Certified True Copy of parents’ passport (must be authenticated by the nearest Philippine Embassy or Consulate General)
* Original and photocopy of the passport of person traveling with the minor.
* Original valid ID w/ signature or passport of the authorized representative.

If you are a Muslim applicant whose birth was not registered:

Please take note of the following:

* Late registered Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO).
* Original and photocopy of Voter’s Affidavit or other supporting documents indicating date and place of birth and citizenship, identity and tribal affiliation.
* Certificate from the Office on Muslim Affairs (OMA).

If you are a Muslim convert:

Please take note of the following:

* Submit annotated Birth Certificate (BC) in Security Paper (SECPA)
* Court Order granting change of name
* Shari’ah Court Order
* OMA Certificate of Conversion
